Lack of Psychological Safety at Boeing

An excerpt from Jonathan Smart’s Sooner Safer Happier, first published in the DevOps Enterprise Journal. It retells what Boeing’s own quality managers and whistleblowers reported from the 787 Dreamliner and 737 MAX factories: defects flagged and then played down, inspectors pressured to stop recording problems, and warnings about schedule pressure that went unheeded before the crashes.

The lesson is that psychological safety is not a nicety: when the people who see the problems are penalized for saying so, the organization stops learning, and the cost arrives later.
